The active player will end up drawing two cards.

Sinister Concoction's “Put the top card of your library into your graveyard” and “Discard a card” are two different costs that are paid separately. As such the cards involved will be going to the graveyard at slightly different times, and each will trigger The Gitrog Monster when it does.
