Not usually upgrading FtMGS?

This line from the IPG:

2.6. Game Play Error — Failure to Maintain Game State

Judges should not usually upgrade this penalty, as players will be reluctant to call a judge if they believe that they could receive a significant penalty as a result.

What does it mean?

Not usually upgrading FtMGS?

It means that even after 3 FtMGS we don't upgrade it to a game loss. Really this penalty is just to let players know that they need to pay attention, not to effect the outcome of a match because they missed an error their opponent committed.

The “usually” is there because, even though it's quite unlikely, we acknowledge that players sometimes do things that we can't imagine when we write policy. In all the scenarios we could dream up, FtMGS is sufficient to get a player's attention and explain the need to pay attention.

Not usually upgrading FtMGS?

FtMGS can also be a good aid in tracking if a player makes repeated
“innocent” omissions enough times that it no longer appears innocent.
