This one might be incorrect:

Q: Amy controls Everlasting Torment and Form of the Dinosaur. She deals 15 damage to her opponent’s Grizzly Bears. How much damage does Amy take?

A: Two. Form of the Dinosaur and the creature deal their damage at the same time. Though the creature will be smaller afterwards, when the damage is dealt, it has 2 power.

Separate verbs means separate, sequential actions in Magic. “and” versus “then” is purely editorial and has no difference from simply starting a new sentence.

I.e., Grizzly Bears takes wither damage before dealing damage according to its new power.

'O'fficially confirming that David Poon is correct, for the reasons stated.
Since it uses the word “deals” two times, the damage is sequential, not simultaneous, so something like Everlasting Torment (or, more standard-relevant, Soul-Scar Mage) works well with the Form: the Form deals 15 damage to it, but it gets 15 -1/-1 counters on it, then the creature deals damage to you (which probably results in no damage, since it currently has 15 -1/-1 counters on it).
