I'm a little unclear on the specifics, and have two examples I'd like to cite:

1) Anson records he is playing 60 cards. Upon a deck check, it is discovered that the cards he has written down total 61, and when examining his deck, it is found that two copies of Hazoret's Monument are listed on his decklist, but only one is present in his deck.

2) During a Top 8 deck check, it is found that Noah has listed 4 copies of Wandering Fumarole, but in his deck are three Wandering Fumarole and a Highland Lake. Highland Lake is not listed on his decklist.

Would these be deck problems or decklist problems?
