Him thinking the first one was for a missed trigger? an honest mistake.
Now you go over the original GRV to see what it was for, and find out it was a wrong call by the judge issuing it, and that no appeal was made.
How do you proceed?

The game is over, retroactively applying a game loss is just bad all around, and should not be done.

If you want to check out if a player should be receiving a Game Loss for accrued penalties, this is definitely something that should have more time investment - go and check with the scorekeeper, who hopefully will be on top of penalties by this point, and take a few minutes to decide on your action. The rounds are untimed in the single-elimination portion, and for such a case, you probably want to be sure about your actions.

As described, you're only issuing a potential second warning, so it's not upgrading to a Game Loss right now (and it might not even be relevant), but making sure so that you are ready and prepared for that eventuality is a good option.

If a judge has made a wrong call and issued a penalty wrongly, that's not too big of a deal either. There seems ample opportunity for education, here, and if necessary/possible, an apology to the player.

Judge error here, I'd say. When the player tells you he has a previous penalty for the same thing, judge should check. Players may well not know the technical distinctions between penalties, such as GRV or MT.

As for the fix: Nothing to do. The judge made the wrong call by issuing the last warning, it's done. Explain to the player about it so he understands that future ones may well be GL. Discuss with responding judge about checking penalties when players tell you they have some.

Complacency can be bad.

Furthermore, we shouldn't upgrade a “third” GRV based solely on the player's recall of his or her previous Warnings. Again, check with the scorekeeper for details, then present that to the Head Judge. You'll also want the players to keep playing, not wait for you - in case there's no action required, don't waste round time - so you'll need to check and discuss an upgrade in a timely manner - in case there IS action required, before the match progresses too far.
