Deck problem - 5-of

Yesterday, during a deck check, I found a deck with a physical 3x - 5x.
Decklist was correct, 4x - 4x.

(Constructed tournament, obviously)

I gave a GL and we fixed the deck.
Wondering what is the fix if this error is discovered during a game, i.e. “Judge! I just drew the 5th copy of this card”.

I would say, you lose the drawn card, Warning, keep playing. Is this correct?

Or this upgrade clause applies?

“If an error resulted in more copies of a main deck card being played than were registered and this was discovered after the game had begun, the penalty is a Game Loss.”

What do you mean by “a physical 3x - 5x” and “4x - 4x?” Does this mean the deck, for example, had 3 Shock and 5 Magma Spray instead of the listed 4 Shock and 5 Magma Spray?

This is a deck problem. The penalty is a game loss, as per the allowed
upgrade:
“Upgrade: If an error resulted in more copies of a main deck card being
played than were registered and this was discovered after the game had
begun, the penalty is a Game Loss.”
