The procedure for reporting disqualifications is changing as of today. DQs should now be reported through a Google Form. Please see this blog post for details: https://blogs.magicjudges.org/judgeapps/2017/03/20/submitting-investigations-on-judgeapps/

A link to the form can be found here: https://apps.magicjudges.org/investigations/ . There is also an alternative reporting procedure for those who can't access Google Forms.
